Dr. Sari Eitches is an independent physician in private practice. Care is self-pay (out-of-network); we do not bill insurance directly. This practice model allows for thorough, personalized care without the constraints of insurance-based visit structures.

Our office values transparency in pricing. You pay for the clinical support you need. To remain an active, current patient in the practice, an annual preventive health visit is required every 12 months. Follow-up frequency varies by individual need: some patients benefit from visits every 1–3 months (for example during active workup and treatment changes), while others are seen once yearly.

In addition, active patients are billed a $300 annual administrative fee each January. This supports the between-visit work required to care for patients—secure messaging, prescription refills outside of visits, prior authorizations, forms, coordination of care, and communication with other members of your medical team. This fee is separate from visit fees, labs, and imaging.

Payment & Reimbursement
Fees are collected at the time of service. We accept cash, check, credit card, Zelle, Venmo, and eligible HSA/FSA accounts.

We can provide a superbill for patients who wish to submit to a PPO plan for potential out-of-network reimbursement. Reimbursement varies widely by plan; we recommend contacting your insurer to confirm your out-of-network benefits.

Labs & Imaging
If labs or imaging are recommended, you may use any lab or center you prefer to maximize insurance coverage. For patients without insurance, we can often arrange pre-negotiated cash pricing for lab testing.

Cancellation Policy
Please note: we require 24 hours’ notice for cancellations or rescheduling. Because Dr. Sari runs a low-volume, time-intensive practice, late cancellations are subject to a fee.
