Dr. Sari Eitches is an independent physician. She is out of network with all insurance companies. She is not affiliated with any hospital, pharmaceutical company or lab. This allows her to provide exceptionally high quality patient care without influence or restrictions.
Our office values transparency in our pricing. You pay for the clinical support you need. There are no membership fees. To remain status as an active, current patient, an annual preventive health visit is required every 12 months.
This fee schedule reflects what is collected at the time of service. We accept cash, check, credit card, zelle, venmo and HSA/FSA healthcare flexible spending accounts.
We provide a superbill that you can submit to your PPO insurance company for reimbursement. The amount that your insurance will reimburse varies greatly from plan to plan so we recommend calling your insurance plan to inquire about out of network benefits.
If labs and imaging are recommended, they can be performed at any lab or center you prefer to maximize insurance coverage. If you do not have insurance we have arranged pre-negotiated cash prices for lab testing.

New Patient Initiation fee and Initial Visit and Physical Exam $950 (60-90 minutes)
Annual Preventive Health Visit $600 (60 minutes)
*including physical exam when done in person
Annual Physical plus Pap $750 (90 minutes)
Complex follow up Visit $600 (60 minutes)
Standard follow up Visit $350 (30 minutes)
Brief, targeted Visit $250 (15 minutes)
Sick Visit (same day or next day appointment) $350 (30 minutes)
*includes point of care testing when indicated such as rapid strep, flu and covid testing and send-out cultures.
After Hours care by phone or email for new/urgent medical issues $150+
